Morning of Surgery/Procedure Admission Form

Please complete this form the morning of your pet’s surgery or procedure, before dropping them off at our clinic.

Post-Surgery Assessment Form

Please complete this form 10-14 days after your pet’s routine surgical procedure (such as spay or neuter), and before any post-surgery recheck appointments. This will help us evaluate if your pet has healed well and can go back to their normal routine.

Parasite Prevention Request Form (Flea, Tick, Heartworm & Intestinal Worms)

Please complete this form to request preventative medication for your dog or cat, to protect them from fleas, ticks, heartworms, ear mites and intestinal worms.

Prescription Refill and Food Order Request Form

You are welcome to use this form to request a prescription refill or a specialty veterinary food for your pet, rather than ordering by phone or email. One of our team members will contact you when your order is ready. Please note that some prescriptions cannot be safely refilled without examining your pet – if necessary, we will contact you to arrange an appointment. For urgent prescription refill and food purchase requests (within 3 business days) please contact us by phone at 416-537-9677, and be aware that a rush fee may apply. Rush fills may not be possible for special order and compounded medications.

Acupuncture New Patient Questionnaire

Before your pet’s Acupuncture consultation, please complete our new patient questionnaire. This will allow the veterinarian to review their case and prepare for your first visit.
