Veterinary Technician Pre-Appointment Form

IMPORTANT INFORMATION: PLEASE READ!

You have an upcoming appointment with one of our veterinary technicians. Your pet will NOT be seen by a veterinarian.

Veterinary Technician appointments are for minor, routine procedures, such as:  

1) Nail trims
2) Blood pressure testing
3) Prescribed blood draws & urine collections that don't require an accompanying exam by a doctor
4) Injections of prescribed medication
5) Weight rechecks & nutritional consultations, etc.

Veterinary technicians cannot address medical concerns in these appointments, and if these arise, it will NOT be possible for your pet to also see a veterinarian during the same appointment. If your pet is sick or injured, has new or existing health concerns, is due for vaccines, or is in need of a wellness or recheck exam for any reason, a separate appointment with one of our doctors will need to be booked.   

If your pet's appointment should NOT be booked with a veterinary technician based on the criteria above, or if you are unsure, please call the clinic as soon as possible, and we can advise if you should rebook your pet with a veterinarian. 

Does your pet live with anyone that has tested positive for COVID-19 in the last 10 days or has a fever, respiratory symptoms, loss of smell or taste, conjunctivitis or other contagious illness symptoms?*

If yes, please arrange for someone outside your home to bring your pet in on your behalf, and call us right away if that is not possible, so that we can make a plan together. This is essential so that we can protect our team members and ensure that we can stay open to help pets in need.

If you do not know the name, please describe what it is being used for.
Has your pet been experiencing any of the following: vomiting, diarrhea, coughing, sneezing, discharge OR changes in eating/drinking, energy, urination or defecation? Or do you have any new or existing medical concerns or questions for your pet that you would like addressed?*


If yes, they may require an exam with a veterinarian, unless they have just had one and are coming in for a follow-up technician treatment that the doctor prescribed. Veterinary technicians cannot address medical issues - if in doubt, please contact us to check if you should rebook with a veterinarian.

If your pet is on long term medication for issues like thyroid disease, diabetes, Cushing's, Addison's etc., it is very important that you let us know if you have missed any doses in the past 4 weeks, as in some cases this will make bloodwork results inaccurate, and could require them to be repeated again (at additional cost).

 

We require calming medication for Blood Pressure checks to get calm accurate readings. Some pets need calming medication to allow us to safely take blood samples or trim their nails. If we have prescribed or recommended calming medication for your pet in the past (or you feel it will be helpful), and you do not have a current prescription at home, contact us immediately.
